Confidentiality Statement

This agreement should be signed by all attendees of the GENYSIS sign-out meeting. 

I acknowledge that confidential, proprietary, and/or other sensitive information, including that which may be protected by federal and state privacy laws and institutional security policies, may be distributed or discussed in connection with the review activities of the GENYSIS core service of the University of North Carolina at Chapel Hill (UNC). Except to the extent disclosure may be required by law, I agree and affirm that I will hold in strictest confidence all information I receive during attendance at GENYSIS sign-out meetings. 

When participating in GENYSIS activities, including variant analysis and/or sign-out meeting, I also agree to the following: 

• I will not save proprietary GENYSIS documents (e.g., study protocols, patient results) to portable devices (e.g., laptops, jump-drives) unless specifically allowed by study protocol. 

• I will maintain a secure environment when reviewing GENYSIS documents in public places (e.g., cafe, airport), including maintaining possession of my laptop at all times and shielding my screen from the view of others.
